Eligibility Requirements
The recipients of the services:
- Must need either skilled nursing care, speech therapy, physical therapy, or occupational therapy on an intermittent basis
- Must be restricted in their ability to leave their home
- Must have the home-based care prescribed by the physician

- Muscle Weakness
- Problem Walking
- Balance Issues
- Difficulty Speaking/Swallowing
- Difficulty Dressing
- Recent and Frequent Falls

- Ask your physician for a prescription, stating your diagnosis and what ideal home health care services you need.

Referrals will be accepted from physicians, hospital discharge planners, families and friends, and private individuals.
