Eligibility Requirements

The recipients of the services:

  • Must need either skilled nursing care, speech therapy, physical therapy, or occupational therapy on an intermittent basis
  • Must be restricted in their ability to leave their home
  • Must have the home-based care prescribed by the physician

  • Difficulty Speaking/Swallowing

  • Recent and Frequent Falls

Referrals will be accepted from physicians, hospital discharge planners, families and friends, and private individuals.
